    I have been tasked with developing a central receiving system plan for a large county that includes multiple out patient providers that are not attached to the acute care part of our system. The community is interested in financing a facility based model that may include multiple providers working on care coordination teams . While it would be great if one facility could triage everyone it is not practice due to transport issues and the size of the county. Has anyone used a central receiving model that includes telehelath/ virtual assessment , triage and care coordination through teams staffed by multiple providers? I'm concerned about the practicality of this and potential risk factors. Any advise will be very appreciated.
