    Our CMHC is updating our Mortality Review protocol, and I'm interested is hearing from other agencies about what your practices are.  I would welcome any information.  Also, we're very interested in ways to more closely connect lessons learned from these reviews back into clinical practice.  Thank you.

    Hi Emily,
    We also recently redid our PNP to support clinical connection with the MMR.  It was a bit scary for all types of providers at first as most of them had not had experience with that kind of process, but but it has been helpful and well received.  Additionally, it has provided us with a mechanism to drive home the importance of patient centered safety in such issues as ensuring that necessary labs are done prior to refilling medications and reinforcing team structure.  I'd be happy to share our PNP and/or discuss off line if you want to reach out.
