  • 1.  does 65% productivity standard include or exclude PTO/holidays?

    Hello,

    I have heard that a common standard for psychiatry productivity is a goal of 65% of time spent providing billable services.  A couple of questions, for those that have implemented this, does this include or exclude PTO/holidays, and how successful have you been?

    I worked at an agency that implemented this standard with their therapists. My observation was that it was extremely counterproductive in three important ways. 
    1) It led to tension between the agency and their employees especially those who worked with children who felt they were being punished for doing important work like coordinating with other providers, coordinating with schools. talking with parents, no shows, people arriving late for their appointments, researching information and resources to support people, consulting with colleagues, and attending meetings and trainings.
    2) It led to conflict between clinicians and their supervisors who felt unappreciated when they were told they were "unproductive" despite their hard work.
    3) Within 1 year all of the child therapists and 80% of the entire staff had left. This left a department that was more "productive" while also being less effective.
    At that agency they calculated by adding up the number of billable minutes (pulled from the EMR) and dividing it into the amount that person was paid for in a given time period.

    Psychiatry is one of our highest cost centers and the longest wait list for services, therefore we must be as efficient as possible to increase service to our community and reduce overall costs.  Our psychiatric providers, for the most part, reach this expectation and like their clinical therapist peers who have 50% productivity standards, receive bonus incentives for over production as a result of efficient scheduling and complete, timely documentation.  The providers love this plan, they are fully in control of extra money they can make and, if efficient, can add several thousand more to their base salary on an annual basis.  This is a win win for the staff member and the organization as it is not a fixed salary cost I need to build into our budget.  Our retention rates have increased as a result of this program.

    Hi Brian,

    Our psychiatry providers have a 60% productivity expectation that is inclusive of vacation and holidays, 1224 annual hours expected of the 2040 available work hours.

    ​We have always set our minimum expected billable hours at 65% of "worked hours".  From a  budget perspective, our actual target is an annual total of 1200 billed hours.  We use the 65% standard as a means to track performance from week to week and month to month.  Sixty-five percent of 2080 is actually 1352, so the metric builds in an average amount of leave time, training days, etc.. We're really firm on holding the line on non-billable client related services for which clinicians think they should be given "credit.". We calculate billed time based on the CPT codes with new patient intakes earning 1.5 hours for a billed session and groups calculated .3 hours per participant billed hour. 

    If folks have other ways of calculating productivity or alternate minimum expectations, I'd be very interested.
